Accident/Incident Reporting and Investigation Procedures

PURPOSE

To establish procedures and responsibility for investigating accidents and incidents at all sites. The intent of the investigation is to establish facts and circumstances, determine causes and assign countermeasure action to minimize any reoccurrence.


To formalize a process and reassure employees that Canadian Protection Providers Inc will investigate and correct conditions of hazards to maximize their Health and Safety.

SCOPE

This investigation procedure applies to and will be initiated for all work related accidents/incidents involving Canadian Protection Providers employees which resulted in or had the potential to result in losses such as:

  • Lost time and health care injuries/illness
  • Motor vehicles accidents
  • Fatality or critical injury,

Note: Any part of this procedure may be initiated by company Management, Site Supervisors/Patrol Drivers, or members of the Health and Safety committee.

DEFINITIONS

Accident: an undesired event that results in death, ill health, injury or property damage or loss.
Accident Investigation Report : Russell Security form completed in cases of all reported lost time and health care accidents.
Critical Injury : An injury that:

  • Places life in jeopardy
  • Produces unconsciousness
  • Causes loss of sight in an eye
  • Results in substantial loss of blood
  • Consists of burns to major potion of the body
  • Results in fracture or amputation

Health Care: Injury/Illness requiring off-site health treatment.
Lost time: Work related injury/illness which results in one (1) or more days of absence.
OHSA: Occupational Health and Safety Act.

GENERAL

Russell Security Service Inc believes that accident/incident investigations are critical to preventing Health and Safety incidents and accidents. Effective investigations are essential to determine:

  • What actually happened
  • Contributing factors and root cause
  • Likelihood of reoccurrence
  • Trends involving similar incidents

INITIAL REPORTING OF ACCIDENT OR INCIDENT

Employees discovering a health and safety incident or accident shall immediately report it to their Shift Supervisor/Patrol Driver and/or contact emergency response services if required.

INITIAL SUPERVISORY RESPONSE

The Shift Supervisor/Patrol Driver shall, upon notification of an incident/accident:

  • Immediately proceed to the incident/accident location
  • Take control of the scene
  • Ensure appropriate first aid/medical attention
  • Secure the scene to prevent secondary accidents/incidents from occurring
  • Preserve evidence
  • Obtain names of witnesses
  • Inform company management

POST-INCIDENT/ACCIDENT ACTIVITIES AND INVESTIGATION

The incident/accident investigation shall be conducted by the Site Supervisor/Patrol Driver or designate after the incident scene is deemed safe.
When conducting witness interviews the investigator shall:

  • Obtain statements from the wittiness in their own words
  • If the incident/accident results in a critical injury (per Section 3.3) statements must be made in writing and signed by the individual
  • Use visual aids such as pictures, sketches etc. to clarify points.

REPORT WRITING

The Site Supervisor/Patrol Driver or designate shall complete a Russell Security Investigation Report for all occupational incidents resulting in employee Medical Aid or Lost Time injuries and/or incidents involving:

  • Fire or explosion
  • Motor vehicle accidents
  • Other incidents of significant nature.

The Accident Investigation Report (written) shall include:

  • Description of the accident/incident
  • Identification of the probable causes
  • Temporary and permanent countermeasures to prevent reoccurrences

FATALITY OR CRITICAL INJURY INCIDENTS

When a incident results in a fatality or critical injury the following additional steps shall be taken:

  • Russell Security Management will be contacted and made aware of the incident immediately
  • At least one (1) joint Health and Safety committee member (certified and elected) shall be contacted immediately and shall initiate an investigation separate from the company investigation (OHSA 9.31)
  • Russell Security senior management shall determine if the injury is reportable to the Ministry of Labour

If the injury is fatal or critical, senior management will decide who will inform the Family/Next of Kin.

FOLLOW-UP AND REVIEW

  • Russell Security management shall review the details of the completed report to determine if the incident/accident had the potential for extremely severe injury.
  • Site managers / Supervisors are responsible for ensuring that countermeasure activity is assigned, tracked and audited.
  • Original investigation report/forms will be maintained on site.
  • Copy of investigation report to be sent to the Joint Health and Safety Committee for review.
  • All documents will be maintained and archived according to Russell Security record retention policy.

RESPONSIBILITIES

Russell Security management and the Joint Health and Safety Committee will review and update this procedure annually.
